Requirements for New Patients
1. You must have a Valid NM Driver’s License/ID (required by NMDoH)
2. Below is the list of approved conditions, you must have at least one.
Amyotrophic Lateral Sclerosis (ALS), Cancer, Crohn’s Disease, Epilepsy, Glaucoma, Hepatits C infection*, HIV/AIDS, Huntington’s Disease, Hospice care, Inclusive Body Myositis, Inflammatory Autoimmune Mediated Arthritis, Multiple Sclerosis, Damage to the nervous tissue to the spinal cord *, Painful Peripheral Neuropathy, Parkinson’s Disease, Post-traumatic Stress Disorder, Severe Chronic Pain, Severe Anorexia, Cachexia, Spasmotic Torticollis (Cervical Dystonia), Ulcerative Colitis, Obstructive Sleep Apnea.
* Require additional proof of condition
3. You must provide medical records related to your condition (no summaries)
How to request your medical records
We recommend that you request your medical records in person at your medical clinic or provider’s office. Requesting your medical records in person is the fastest way to get your records.
You will need to fill out a release of information form with your clinic or you can download our medical release form. to take with you.
