New Patient Cannabis Card Information
This patient guide is the first step to obtain a medical cannabis card in New Mexico. We welcome all patients who are new to the New Mexico Medical Cannabis Program.
Requirements for New Medical Marijuana Patients
You must have a Valid NM Driver’s License/ID
You must have an approved medical condition
You must provide medical records showing that you have an approved medical diagnosis (preferably within the past 12 months with at least 2-3 notes pertaining to your diagnosis) No after-visit summaries.
The easiest way to get your medical records is to request the records in person at your medical clinic/provider’s office. It’s very important that you request your records in person. Also let the person preparing your request know you need your medical records for an upcoming medical appointment. Requesting your records in person is the most convenient method and we recommend this.

Severe Chronic Pain
Spasmotic Torticollis (Cervical Dystonia)
